Once you have re-entered your password and have clicked the Continue button, the Voucher page displays:

1.		Enter your voucher number in the Voucher Number field.
2.		Enter your name in the Name field.
3.		Enter your location information in the following fields:
		Street   City   State   Zip Code   Phone
4.		Click the Submit button to proceed.
