TCC2P cards (800-24766-05 and earlier) continually reset, requiring the card to be physically removed and re-inserted into the slot to recover. Although this issue is most susceptible on 4.0.0 - 6.0.1 software releases, it is possible to see this issue on 4.0.0 - 8.5.2 software releases.
Several customers running 4.x.y releases reported multiple instances with TCC2P equipment failing in the field.
This problem was observed on TCC2P cards running older software releases (4.0.0 - 6.0.1).
Note: It is possible to see this issue with releases 4.0.0 - 8.5.2.
The TCC2P continuously resets in the shelf and eventually reports an equipment failure.
The only way to recover from this equipment failure is to physically re-insert that card back into its slot.
Resolution to this problem can be achieved with a change to the boot code in the TCC2P. Version 800-24766-06 and higher TCC2Ps already have the new boot code. Version 800-24766-05 and lower TCC2Ps installed in the field may be upgraded either through a software upgrade, or using the Bootrom Patch Tool that only updates the TCC2P bootcode.
Information about the Bootrom Patch Tool: This patch tool is functional with software versions 4.0.0 through 9.0.0 only. Once the tool is used on a card, the card will not revert to the original bootcode version when using any of these releases. The tool updates bootcode on both the active and standby TCC2Ps.
Initially v1.0 of the tool was made available. Recently a new version (v1.2) is now available which adds retry capability to insure a smooth upgrade process.
The following action is recommended based on various field configurations that may exist:
1) Card versions 800-24766-06 and later:
No action required. Regardless of software release, these cards are not subject to this failure mode.
2) Card versions 800-24766-05 and earlier running release 4.0.0 through 8.5.2:
Action required. To avoid this failure mode in 800-24766-05 and earlier versions of the card, the boot code may be updated in the field. The "ONS 15454 TCC2P Card BOOTROM PATCH TOOL 1.2" is available with the capability to upgrade the boot code (New boot code 57-5444-09). See the How to Upgrade Software section for specific details.
3) Card versions 800-24766-05 and earlier running release 8.5.3 through 9.0.0:
Release 8.5.3 through 9.0.0 contain the new boot code for the TCC2Ps. These releases will automatically upgrade the TCC2P boot code and therefore don't require the patching utility to be run.
However, if the cards running these versions are later used in a system with a 4.0.0 - 8.5.2 software version, they will revert back to the previous version of boot code unless the patch utility was run on them when they were in the 8.5.3 - 9.0.0 node. If the patch tool was not run, it may be executed after installing in the 4.0.0 - 8.5.2 system.
Hardware Identification from within CTC:
The hardware can be identified through Cisco Transport Controller (CTC). To determine the Hardware Part Number of your card do the following:
Open Cisco Transport Controller.
From the Shelf View, select Inventory
From the Inventory tab, locate the particular card of interest and compare the information from the Inventory tab against the Product information within this field notice.
Note: The inventory screen shot below is provided as example only. Equipment type, part numbers and other information may differ.
A specific step-by-step procedure for running the ONS 15454 TCC2P Card BOOTROM PATCH TOOL 1.2 is fully described in the README file for the Windows and Linux operating systems.
Note: All references to v1.0 of the tool in the README files should be ignored as the new version of this tool is v1.2
If v1.0 has been previously copied, the new v1.2 should be copied to a new directory and executed from this new directory.
A compressed file is available for each operating system (Windows and Linux). Each compressed file contains the Utility and the upgrade procedures (README File) for successfully upgrading the boot code on the TCC2P cards. The compressed files can be downloaded for the appropriate operating system from the Cisco Web site:

3) Select a Software Type:
Optical Networking Systems (ONS) Firmware Patch Tools
4) Choose the 1.2 folder.
5) Select Windows or Linux compressed file for download.
For Windows OS, the compressed file name is:
"ONS 15454 TCC2P Card BOOTROM PATCH TOOL 1.2 (for Windows), containing v09 Bootcode (57-5444-09); For use with TCC2P Card Only."
For Linux OS, the compressed file name is:
"ONS 15454 TCC2P Card BOOTROM PATCH TOOL 1.2 (for Linux), containing v09 Bootcode (57-5444-09); For use with TCC2P Card Only."
Follow all steps called out within the appropriate README file for successful boot code upgrade.
