Problem Description
After a Catalyst 6509-NEB switch is upgraded to use the Supervisor Engine 720 with the WS-6509-NEB-UPGRD upgrade kit, the 6509-NEB-FAN2 fan tray fails upon bootup.
All affected 6509-NEB-FAN2 fan trays will fail upon insertion.
All LEDs of the Power Supply look good, but the INPUT POWER GOOD LED of Fan Tray is not lit.
This is a summary of the LEDs:
Power Supply LEDs
INPUT OK LED: Green FAN OK LED: Green OUTPUT FAIL LED: Not lit 42V LED: Green
Fan Tray LEDs
INPUT POWER GOOD LED: Not lit <===== FAN FAIL LED: Red
Background
After a Catalyst 6509-NEB switch is upgraded with the WS-6509-NEB-UPGRD upgrade kit to faciliatate the supervisor engine 720, the 6509-NEB-FAN2 fan fails due to reversed polarity of its terminal block.
Power is not supplied to the fan tray due to the reversed polarity, and the INPUT POWER GOOD LED of the fan tray is not lit.

Workaround/Solution
Workaround: There is no workaround.
Solution: In the event that this type of failure is encountered, failed 6509-NEB-FAN2 fan trays should be returned to Cisco via the normal RMA process. Please use the WS-6509-NEB-UPPGRD part number for the RMA. WS-6509-NEB-UPPGRD includes the WS-C6509-NEB-FAN2.
How To Identify Hardware Levels
Affected fan trays can be identified by a white CLEI code sticker on the front panel. It will will be marked with the following data: 118792 / CNDQASBGAA
