Problem Description

An issue has been identified at customer sites and verified through Engineering Failure Analysis (EFA) indicating a vulnerability with three under-rated electrolytic capacitors positioned at the -48V input on the 15540-FTMP. Failure of one or more of these capacitors can result in a failure of all eight fans within the 15540-FTMP assembly. For a fully populated chassis, system assemblies may begin failing 15 minutes after a 15540-FTMP failure. A sparsely populated chassis with a low ambient temperature may never reach critical temperature.

Background

This issue was first seen at a customer site and has been verified through multiple EFA's of the 15540-FTMP.

Problem Symptoms

After a 15540-FTMP has failed, messages will be sent to the console logs of the 15540 reporting FAN_MISSING. The 15540 Chassis may reach critical operating temperature, resulting in component failures throughout the 15540 system. The time it takes to reach critical operating temperatures is dependent upon chassis hardware configuration, service bit-rates and ambient temperature. For a fully populated chassis, components may begin failing 15 minutes after a 15540-FTMP failure, while a sparsely populated chassis with a low ambient temperature may never reach critical operating temperature.

Workaround/Solution

To address this issue, it is recommended that all 15540 customers using the 15540-FTMP HW versions prior to the 800-09381-05 request RMA replacements for the 15540-FTMP using the standard RMA process to avoid the high potential of a network impacting 15540-FTMP failure.

When requesting an RMA for this product, Chassis serial numbers must be included with your request. To determine the Chassis serial number, use the following commands in privileged EXEC mode:

("CHS" will limit the output returned from the show hardware command to include only information on the 15540-CHSA and 15540-CHSB product identifiers.)

Note: In the event of a 15540-FTMP failure prior to receiving replacement material, customers should remove the 15540-FTMP from the chassis, wait for a minimum of 10 seconds and then reseat the 15540-FTMP into the chassis. If this does not restart the 15540-FTMP, customers will need to maximize venting to the chassis to prevent component failures until a spare can be obtained by requesting an RMA directly from the Technical Assistance Center. If the 15540-FTMP recovers after this operation, the customer should still request a 15540-FTMP replacement using the standard RMA process to avoid risk of future failure.

The following are some suggestions on maximizing air venting to the 15540 Chassis:

1. Remove the 15540-FTMP from the chassis

2. Power off all unused transponders and daughter cards using the hw-module subslot power off command in IOS

3. Remove all unused hardware modules and linecards from the chassis

4. Remove all blanks from the chassis

5. Decrease the facility temperature to reduce ambient temperature

6. Ensure that bit-error monitoring is enabled for the traffic on local and remote interfaces to detect bit-errors which may indicated an impending hardware failure

Note: Do not enable the environment monitor shutdown fan command. If this is enabled the linecards may begin to shutdown within 1 minute of the 15540-FTMP failure (up to IOS Release 12.2(25)S3).

How To Identify Hardware Levels

How to determine the 800- Assembly Version of an In-Service fan tray on the ONS15540 :
1. Issue the show hardware command in IOS, if the IOS version is 12.2(26)SV or later.
If the Hardware Version of the fan tray is displayed as 3.0 or higher, then module is a 800-09381-05 or higher. The Version is -05 or higher.

2. If the Hardware Version displayed is lower than 3.0, or if the IOS being used is an older version which does not display the fan tray hardware version, then proceed to visually determine the version as below.

-- Check to see whether there is a 800-09381-xy label on the fan tray front panel.
i. If Yes, the version of the fan tray is the last 2 digits of this number.
ii. If No, proceed to decode the version as below :

-- Read the CLEI code on the label on the fan tray front panel.
i. If CLEI code is WMMYAFSGAC, it is a 800-09381-04 part, and the Version is -04.
ii. If CLEI code is WMMYAHHGAA , it is a 800-09381-05 part, and the Version is -05.
iii. Otherwise, it is a 800-09381-03 or earlier part, and the Version is -03 or earlier.
