Problem Description
Two problems have been identified in MDS 4.2(1).
The first issue occurs when installing the SSI image onto a SSM using the install ssi command. The port manager process crashes, which causes a failover to occur. The port manager process continues to crash, which causes the switch to reload.
The second issue occurs in some cases of zone change where unwanted TCAM entries can still persist and the member does not receive change notification.

Background
The problems were both identified in internal testing and are documented in CSCtb47714 and CSCtb65569.

Two issues exist in the 4.2(1) software. The first occurs when installing the SSI image onto a SSM using the install ssi command. The port manager process crashes, which causes a failover to occur. The port manager process continues to crash, which causes the switch to reload.
The second issue occurs in some cases of zone change where unwanted TCAM entries can still persist and the member does not receive change notification.
Workaround/Solution
It is recommended that customers upgrade to Cisco MDS NX-OS Release 4.2(1a) or later and activate the zone set again. If the zone set activation is rejected, you see the message: specified zoneset active and unchanged. In this case, add a dummy member with a non-existing pWWN to any of the zones in the zone set and activate the zone set again.
Cisco MDS NX-OS 4.2(1a) should be available on September 25, 2009.
