Problem Description
Cisco has identified that Software release 12.2(25)EWA10, 12.2(31)SGA2 and 12.2(31)SGA3 will erroneously report some WS-X4548-GB-RJ45V PoE cards as faulty on bootup. Linecard PoE interfaces will flap and the connected PoE device(s) will power up and down continuously.
Background
WS-X4548-GB-RJ45V hardware revision 4.0 is affected by Health Monitoring code for PoE in 12.2(25)EWA10, 12.2(31)SGA2 and 12.2(31)SGA3 only.
PoE Health Monitoring Diagnostic software incorrectly reports PoE errors for module WS-X4548-GB-RJ45V, hardware revision 4.0. The software resets the PoE ports continuously, and the module becomes non-operational.
Problem Symptoms
PoE devices and data connections become non-operational if the WS-X4548-GB-RJ45V hardware revision 4.0 is used with IOS 12.2(25)EWA10, or 12.2(31)SGA2, or 12.2(31)SGA3. Linecard Interfaces will flap. Connected device(s) will come up and then permanently go down.

Workaround/Solution
Customers can either downgrade to a prior release or upgrade to 12.2(25)EWA11 or 12.2(31)SGA4 or to a later release to correct this issue.
