Problem Description
A problem has been found with the 128 MB flash disks for the Cisco 7401. When a read operation is performed, the command-line interface (CLI) reports 0 K of memory available at powerup and any files that were on the flash disks are no longer visible. If the disk is accessed while the router runs, Cisco IOS® Software reports that the disk is of improper format or is not installed. This problem only occurs on flash disks manufactured by Hitachi. This problem does not occur on the Sandisk flash disks.
Background
A problem has been found in one of the field-programmable gate arrays (FPGAs) that causes a latch up problem on the flash disk. During a cold boot, the FPGA latches up the flash disk, which results in corruption of the onboard flash disk controller. The corruption results in failure of the system to access the flash disk. When a read operation is performed, the CLI reports back 0 K available space on the flash disk, and any files that were on the flash disk are no longer visible.
Problem Symptoms
When a read operation is performed, the CLI reports back 0 K available on flash disk. Furthermore, any files that were written to the flash disk are no longer visible to the system. The system fails to boot if the system is instructed to boot from a Cisco IOS Software image on the flash disk. The system boots the boot helper image instead.

Workaround/Solution
If the a flash disk is no longer readable and it is manufactured by Hitachi, please submit a return materials authorization (RMA).
