The GSR16-AC power supply fails with no output.
A failure has been confirmed in the GSR16-AC power supplies, built with Tyco or Lucent supplies manufactured from the twelfth to the thirty-first week in 2000. GSR16 systems shipped between July 2000 and December 2000 could have the defective supplies.
The failure is a breakdown of the low temperature oxide (LTO) layer of the MOS-FET devices, which results in no output from the power supply. The time to fail depends on the thickness of the LTO layer, and it can be from a few months to several years.
Problem SymptomsThe symptom of this failure is no output from the power supply.
There is no workaround for this problem. The solution is to replace the PWR-SGR16-AC power supply.

How To Identify Hardware Levels
You can identify power supplies that have suspect MOS-FET devices by the Tyco or Lucent serial number on the label located on the top of the supply.
Note: The power supply must be removed from the chassis to view this label.
Tyco purchased the Lucent power supply group, so you may see either name on the label. Supplies with serial numbers within the range of 00KZ32000000 through 00KZ51999999 are the ones which were built with the suspect MOS-FET devices. These supplies would have been shipped in Cisco 12016 systems between June and December of 2000.
