Problem Description
When using the Windows NT4.0 NDIS4 driver version 6.64 from the CD shipped with the PCI340 series adapter, the driver does not load properly. After selecting the card and setting parameters, a dialog box informs the user "Setup operation failed" and the driver is not installed.
Background
This issue occurs with Cisco Aironet PCI34x cards that are identified with the "4800" Device ID. The NDIS4 driver (used for Windows NT4.0) only works with PCI cards that have the "340" device ID.
Problem Symptoms
During driver installation process, after selecting the card and setting parameters, a dialog box informs the user "Setup operation failed" and the driver is not installed.
Workaround/Solution

Use the driver PCINT4v664.exe which is posted on CCO: http://www.cisco.com/pcgi-bin/tablebuild.pl/aironet-utils-win (registered customers only)
Download PCINT4v664.exe to a hard drive, and then run it to extract the driver files (preferably to a floppy drive).
Install the driver as detailed in the Hardware Installation Guide (registered customers only) with the exception of Step 4. Rather than pointing to the CD, point to the file OEMSETUP.INF.
