Problem Description
About one minute after the IGX-URM comes up, the IGX8400 registers an IPC failure and declares the URM card "failed".
Background
This anomaly is caused by an uninitialized field which causes IPC communications to fail.
Problem Symptoms
Besides loss of services through the affected IGX-URM, the network engineer will observe that the IGX-URM status (issuing dspcds) is failed and the event log (issuing dsplog) indicated removal of the URM.
The following is a part of the dspcds command output showing a failed IGX-URM:

Workaround/Solution
There is no known workaround. Cisco IOS Software Release 12.2(8)T4 will have the fix for this anomaly.
