ONS 15454 with DS3XM-6
A DS3XM-6 card in an ONS 15454 can reset after running for approximately five months. This can occur with nodes running any software release from Release 3.2.x to Release 4.1.2, or Release 4.5. This issue is resolved in Release 4.6 and maintenance Release 4.1.3.
In an unprotected configuration, the reset should not cause any traffic errors. However, in a 1:1 protection group, the reset triggers a protection switch and might result in an outage if the protect card resets at the same time. Traffic will be restored once one of the cards, either the working or protect card, finishes the reboot process. Depending on the exact timing of the resets, the traffic recovery may occur when both cards finish rebooting.

A monitoring script is available from the Software Download Page (registered customers only) to give you an indication if a reset is likely to occur, so that you can plan a scheduled reset. The scheduled reset should be performed as follows to avoid traffic disruption.
For each DS3XM-6 card pair:
Lock out the protect card.
Reset the protect card. Wait for this card to finish rebooting.
Reset the working card. Wait for this card to finish rebooting.
Remove the lockout.
This issue is resolved in Release 4.6 and maintenance Release 4.1.3.
