 Reason for Advisory:

CSCvx11527 ? Upgrade to the UCS Infrastructure software bundle 4.1(3a) from 4.1(2) or 4.0(4i) or 3.2(3p) reboots both 2408 IOMs simultaneously, causing the chassis to lose network connectivity. This issue affects domain setups where UCS 5108 chassis with 2408 I/O Modules (IOM) connected to UCS 6454 or UCS 64108 Fabric Interconnects.

Affected Platforms:

Cisco UCS B-Series Servers

Symptom:

During upgrade of the UCS 2408 IOM to release 4.1(3a) the first time, all IOMs being updated to the 4.1(3a) release will reboot to update its BIOS without waiting for user acknowledgment. This will cause a complete loss of storage/network connectivity for all chassis using 2408 IOMs.

Conditions:

IOM 2408 updates to 4.1(3a); AND

IOMs in a chassis are updated at the same time, such as using firmware Auto Install process for the Infrastructure (A) bundle

Further Problem Description:

This issue is related to a BIOS update integrated into the IOM 4.1(3a) code. The IOM will update the BIOS when the new image is installed without waiting for image activation action. This BIOS update involves a spontaneous reboot. Given that the image install happens to both IOMs in a chassis simultaneously, the reboot will cause the chassis to lose connectivity completely.

Workaround/Solution:

The UCSM 4.1(3a) release has been deferred. If the IOM 2408 has already been upgraded to 4.1(3a) release, then that IOM has already received the updated BIOS and will not incur further reboots. For continuity, plan to upgrade to a future patch release once one is available.

If the 4.1(3a) firmware was downloaded prior to its deferral, but has not been installed yet, do not to upgrade to this release.
