High-Deductible Medical Insurance
You pay all costs until you reach the annual deductible. Then, the plan covers 100 percent of your eligible expenses for the rest of the calendar year.
The deductible does not apply to preventive care. It is covered at 100 percent.

The deductibles are higher in the HPSP than in Cisco's other health plan options, but employees who enroll in the plan for 2012 will receive a pre-tax contribution to their health savings account (HSA). This contribution can be used to offset the cost of the deductible.
The IRS limits total annual HSA contributions: $3,100 for an individual or $6,250 for a family. All health incentives you and your spouse/domestic partner earn will be deposited in your HSA. Keep this in mind when you're deciding how much to contribute through a pre-tax payroll deduction. (Note: Some states, including California, New Jersey, and Alabama, will apply state income taxes to your HSA contributions.)
Don't get caught by surprise: All non-preventive health care costs are subject to the deductible, including prescription drugs, doctor's office visits, and mental health care. This means that you will pay the full cost of these (and all other non-preventive services) until your deductible has been met.
Single and family deductibles are separate. No individual in the family has satisfied a deductible until the entire family deductible has been met. Once the family deductible is met, the plan will pay benefits for all eligible family members.
