A design flaw has been discovered in the Digi International MIL-100TX MII Transceiver REV. D or lower that can cause damage to Cisco hardware.

In the Digi International MIL-100TX MII Transceiver +5 volts is internally shorted to the case of the transceiver. When this transceiver is connected to any mating MII connector a direct short from +5 volts to system chassis ground occurs and hardware damage can occur.
This problem was first seen at a customer site. Unfortunately, Cisco was unable to get the failing hardware back to perform failure analysis. The second failure occurred in one of Cisco's labs. The failing hardware was taken to engineering for failure analysis and it was determined that the Digi International MIL-100TX MII Transceiver was the problem. All customers using Digi International MIL-100TX MII Transceivers REV. D or lower with the products listed above are affected.
Cisco recommends that all customers refrain from using Digi International MIL-100TX-REV. D or lower MII Transceivers. Digi International has been notified and is currently pulling all of its stock, notifying its distributors, and reworking the REV. D transceivers to eliminate the problem. They will also implement the change on all newly manufactured transceivers. The fix has been verified by Cisco to solve the problem. Customers can now use the Digi International MIL-100TX-REV. E MII Transceivers.
