It has been found that within a few months of usage in a customers network, the optic modules on the cards may fail.
The failure causes loss of traffic in the customer network.
The failure on potentially affected units can occur after only a few months of standard operating conditions.
Reliability tests done on the optical modules of this part by both suppliers have demonstrated the defect.
Assemblies that contain the affected optics module subject to this failure mode are identified by Serial Number in this FN.
The card will eventually permanently fail and cause loss of traffic in the customers network.
Cisco recommends replacing hardware potentially impacted by this issue to avoid a field failure. Customers should follow the standard RMA process to replace the hardware.

As of approximately 5-5-08 new products that were manufactured under Engineering Change Order (ECO) # E093288 are guaranteed to be free of this problem.

In order to determine the Part and serial number of your ONS 15454-10DME-C, 10E-L1-C, or 10ME-L1-C:
Open Cisco Transport Controller, or Cisco Transport Manager:
From the Shelf View, select Inventory
From the Inventory tab, locate the 10DME-C, 10E-L1-C, or 10ME-L1-C cards and compare the information from the Inventory tab against the Products Affected.
If you are using TL1 commands.
Compare the information from the TL1 inventory command against the products Affected section above, and the serial number validation tool.
