Problem Description

DS-n cards were shipped to customers that were built with a "bad lot" of DC/DC convertors.
Manufacturing tests have identified the impacted boards.
A change has been made to the ONS 15454 models listed above to replace a defective power supply which may fail prematurely.
This premature failure may be accelerated at elevated temperatures (50 Deg C). The failure mode exhibits a spontaneous reboot if operational or fails to boot properly on initial install.
No failures have been reported from the field.

Background

Manufacturing found that a DC/DC power convertor used on the 15454, DS1, DS3, DS3E and DS3XM products has a quality issue with certain lot codes. The solder integrity of an internal output filter inductor cannot be guaranteed resulting in degraded and inconsistant performance.

Problem Symptoms

As the power supply warms up on the defective supplies, the 5v side of the supply will drop below an acceptable level and the supervisery circuit on the board that monitors the 5 volts will cause the board to reboot. Cisco Transport Controller (CTC) will indicate an Equipment Failure.

Workaround/Solution

The only workaround is to return the boards for rework to replace the defective power supply.

How To Identify Hardware Levels

Only the listed Serial Numbers above are affected.

If the 800 number is not listed above it is not affected even though the serial number is listed.

Serial numbers do not change when a board is reworked to the latest revision of 800 number.

To determine the Part Number, Hardware Revision and Serial Number of your ONS 15454 DS1-14, DS1N-14, DS3-12, DS3N-12, DS3-12E, DS3N-12E and DS3XM-6 cards:

Open Cisco Transport Controller.
From the Shelf View, select Inventory.
From the Inventory tab, locate the DS1X, or DS3X boards and compare the information from the Inventory tab against the Products Affected section above.
