There is a potential infant mortality component issue which can cause a variety of failures on the ESR-6CT3 line card. More specifics about this failure can be found in the Problem Symptoms section of this notice.
The material used in the packaging of the Line Interface Unit (LIU) devices can fail to provide adequate electrical isolation between the device leads. During early months of operation the component may develop short-circuits internally. If the failure is to occur, it will most likely occur within the first months of operation. If failure doesn't occur within that time frame, the potential for failure decreases.
This is a physical layer problem on the DS3. The T3 controler status changes to down or bounces between up and down.
This condition can additionally manifest itself with one or more of the following symptoms:

The solution to this issue is to replace the failed board. Due to the very low failure rate attributed to this issue (approximately one percent), Cisco recommends a replace on failure strategy. Logistics stock will be purged, but please note that a worldwide logistics purge can take several months. In the interim, to assure that a board is at the latest revision, return materials authorization (RMA) requests for ESR-6CT3s can be coded as Administrative Request - Field Notice (ARFN). This specifies that the replacement is a new unit from manufacturing.

How To Identify Hardware Levels
The revision of the top level assembly (TAN) number of the ESR-6CT3 has been changed. An ESR-6CT3 with a TAN of 800-05547-05 or higher is not affected by this field notice. The TAN of the ESR-6CT3 can be seen in the output of a show diagnostics command as shown in the example below.
